SERVICES
DDA certified; we provide nursing consultation and delegation for developmentally disabled adults in the state of Maryland. We can serve as your delegating nurse for your loved one in the "Self Direction" model; we also train your CMT staff on the nursing care plan, and we collaborate with you to assist you in your needs.​

Pre-Delegation Consult.
This consult is completed either by phone or in person. We listen to your concerns and needs and formulate a plan for your services collectively. This service is complimentary 1 time only.
HRST Documentation
HRST (Health Risk Screening Tool) will be completed as requested/needed.
On-Call Duty
We can provide on-call service to delegate nursing tasks to your certified medication/nursing assitstance staff; We can also offer back-up for your staff delegating nurse as well.
Initial Assessment
After consult is completed and acceptance to delegate; our staff will provide a thorough initial assessment.
This assessment is comprehensive and includes a review of medications, therapies, and brief physical exam; The assessment will be provided to you for your records and needs.
Medical Book.
Whether you need to implement or modify your current book; we can assist you.
Patient-Caregiver Education
Via various forms of learning mediums, we can assist you in various topics to include health, medications, nutrition, etc...
45 Day Assessments
45 day assessments will be scheduled after the initial assessment. These assessments are comprehensive which includes any changes of health, medication, and therapies.
CMT Training
We can assist with delegating nursing tasks to your certified medication technician (CMT) staff. Services includes training on various health topics,medication observances, nursing care plan, and emergency response.
